Registration is open for the MUHC’s My Toolbox Chronic Disease Self-Management Program for the April 2015 session.

Register now for the April session of ‘My Toolbox’, the highly successful MUHC chronic disease self-management program! Workshops begin late April, 2015. My Toolbox offers free 6-week workshops designed to help people learn the skills to live well with chronic health conditions including asthma, arthritis, chronic pain, diabetes, heart disease, multiple sclerosis, cancer, etc. The workshops emphasize coping strategies and practical skills such as: how to manage symptoms, work and communicate effectively with the health care team, reduce pain and fatigue, manage negative emotions such as fear, anxiety, frustration and depression, and get started with exercise and healthy eating. More than 2000 people have registered for this evidence-based program since its inception at the MUHC in 2007.

Registration is now open to anyone living with a chronic health condition.  Family members or health professionals caring for persons with chronic health conditions are also welcome to register.

We run two workshops in either English or French at varying times of the day: Self-Management for Chronic Disease, or for Chronic Pain.  Locations vary from downtown to the West Island, and Westmount). They run weekly for six consecutive weeks, last two and a half hours, and are given.

The workshop is structured and each week different material is presented. Each workshop has 10 to 12 participants. Certified trained volunteers living with a chronic illness themselves facilitate the workshops.
